Whenever one of your employees suffers a work-injury, and where it is practical, THF encourages you
to refer that employee to one of the listed cooperative medical providers. Remember that, you the employer
have the sole and final authority in deciding which medical providers treat your injured employees.

regardless of severity, so THF can help you save an average of 50%+ off every $1 billed. We are

Our website incorporates a simple, interactive tool to quickly and conveniently search our Cooperative
Medical Providers in specific geographic areas who most seem to share our goals of cost effective
healthcare treatment while expediting the work-injured employee’s return to productive work.
If the treating physician intends to refer your employee to a hospital, specialist or diagnostic testing center,
please call THF prior to the services being provided to verify that they are also a cooperative medical provider.
To quickly locate a cooperative medical provider by geographic area and specialty:
1) Enter the city and state or zip code.
2) Select the Provider Specialty.
3) Enter a distance in miles to search for medical providers in your area.
Miles are an estimated calculation. Unless you indicate a larger search area, the search tool provides
the most current information for our Cooperative Medical Providers in your immediate (5 mile radius)
area. By entering a larger number in the Radius block, your search area will be increased to include
medical providers throughout the specified geographic area. Then, just click on the medical provider
name for additional address and telephone information and a MapQuest® link to the medical provider
location.

their standards of care. If you are interested in a medical provider who is not in the directory,
please contact us at 800-716-6777. We can tell you whether that medical provider is not in the
directory because he is reluctant to share our goals or simply because we have had little contact with him.
